We have a number of HP LaserJet printers in common areas that are having a problem that it seems not many people on Google have seen before. When a user prints, a lot of the time what spools is a cover page mentioning "lwow64.exe" (I gather this is probably "splwow64.exe" with the first couple of characters cut off), along with a date-and-time format string that has impossible numbers for the date and time. See this link for a similar format. After that cover page comes a blank page, and after the blank page comes the document.
This would mostly be a problem with wasting paper, except that the cover page and blank page usually have a weird paper type (often "mid-weight letter," but sometimes "envelope letter" or even "custom letter") attached to them.
